COMMUNITY HEALTH ASSESSMENT
The Community Health Assessment (CHA) used quantitative and qualitative methods to collect and examine health status indicators and provide an understanding of health in a community. The CHA guided the development and implementation of a Community Health Improvement Plan (CHIP) by justifying how and where resources should be allocated to best meet community needs.
