coordinated plan of action to respond to

the health needs of the community.

  • Identify our community health priorities

  • Monitor our progress in achieving these outcomes

  • Improve our community’s health

  • Focus our attention and resources on strategies to affect positive health outcomes

COMMUNITY HEALTH ASSESSMENT

The Community Health Assessment (CHA) used quantitative and qualitative methods to collect and examine health status indicators and provide an understanding of health in a community. The CHA guided the development and implementation of a Community Health Improvement Plan (CHIP) by justifying how and where resources should be allocated to best meet community needs.
