Health Literacy Action Team

To provide culturally appropriate health literacy education and improve access to health care in order to prevent disease, increase self-advocacy to services and achieve a healthier Lake County.

Goals & Objectives:

 

Goal 1:

Provide visible, simple health information for residents in their language. 

  • Objective 1: Increased health literacy efforts taking place among Live Well Lake County partners.

  • Objective 2: Strategically coordinated communication efforts throughout Lake County regarding health literacy and medical home promotion

Goal 2 :

Increase number of people who have a Primary Care Physician or a Medical Home.

  • Objective 1: Increase the awareness of preventative benefits (Wellness Annual Visit) offered through health insurance in Lake County.

  • Objective 2: Increased number of referrals made by community based organizations and community health workers to medical homes
